Care Coordinator II (Remote | Central Time Zone)

Schedule: Monday - Friday, 8:00 AM to 5:00 PM CT

Overtime: 5-10 hours/week (subject to business needs)

Employment Type: Full-Time

As a Care Coordinator II , you will support members of the STAR PLUS Waiver population by coordinating essential medical equipment and services that improve quality of life. This is more than just a coordination role-it's a chance to directly impact lives through education, advocacy, and compassionate support.

What You'll Do

* Conduct outreach to members by phone or home visits to review care plans, offer education, and assess needs
* Coordinate DME, medical supplies, and services by working directly with PCPs, DME vendors, and members
* Assist with scheduling, documentation, and tracking delivery and satisfaction of equipment
* Support care transitions by communicating with care managers, providers, and caregivers
* Educate members on healthcare benefits, eligibility, and next steps
* Maintain thorough, compliant documentation according to regulatory standards
* Respond to member inquiries and make referrals to additional support services as needed

What You Bring

Required:

* High School Diploma or equivalent
* At least 1 year of experience in customer service, managed care, or a healthcare setting

Preferred:

* Experience working with DME coordination or within a healthcare system
* Prior remote work experience
* Strong organizational and communication skills
