We are seeking a Community Health Registered Nurse (CHRN) who is passionate about providing high quality primary healthcare to high risk populations, including those in our Healthcare for the Homeless program and other special populations. *About the Role:*
As a CHRN, you’ll use your critical thinking, good judgement, and interpersonal skills as you identify alternative solutions or approaches to patient problems. You’ll report directly to the Senior Director of Quality with the ability to be influential in your role. You’ll collaborate with clinical staff to design and implement meaningful and individualized patient-centered care. Essential duties include:
* Build trusting relationships with patients to facilitate access to health care services and meet health needs
* Engage patients to participate in health promotion, prevention, maintenance and restoration activities
* Assist patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, or other services as needed
* Coordinate and revise the plan of patient care, as needed; identify and address any barriers affecting a patient’s ability to adhere to a healthcare plan
* Conduct community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs, and educate about health needs
You will be primarily based at our Mattina location, 300 Niagara St., Buffalo, NY 14201 and our Bridgeview location, 1050 Niagara St., Buffalo, NY 14213, and will also travel to other Neighborhood sites on a regular basis.

*Education and Experience to be a Community Health Registered Nurse: *
* Degree from accredited 2 year nursing school
* Current NYS Professional Registered Nurse license
* 2+ years of clinical nursing experience
* CPR certification
* Attention to detail, organizational skills, and follow through
* Knowledge of case/care management principles
* Flexible with shifting priorities and timelines
* Strong project and time management skills
* Knowledge of Microsoft Office Suite, Adobe, and database management

Compensation: $36.50 an hour based on a full-time, 40 hour workweek

RN Tuition Reimbursement Potential: Our status as a Federally Qualified Health Center (FQHC) means registered nurses have an opportunity for tuition reimbursement up to 60% repayment of qualified student loans directly related to a nursing degree. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. The reimbursement program requires an application and is not guaranteed.
