The Coordinator of Care RN serves as a community health nurse responsible for managing and overseeing patients' recovery processes at home, including assessment, medication management, and patient/family education. In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right.
Excellent clinical assessment skills
Strong ability to solve problems independently and interact with an integrated team
Current NYS RN license and registration
Bachelor’s degree in nursing preferred
Minimum of one year acute medical-surgical nursing experience
CHHA experience preferred
