This is a remote position where you will be working closely with individuals in your community who utilize the TMG IRIS Consultant Agency. While you will manage your own schedule, the role involves visiting IRIS participants in their homes on most days, ensuring that no two days are the same. As an Eligibility Screener, you will conduct Adult Long-Term Care Functional Screens (LTC-FS) for participants in the Wisconsin IRIS program, a vital Medicaid long-term care option for older adults and people with disabilities. Your responsibilities include carrying out annual rescreens and any change-in-condition assessments using the Adult LTC-FS tool, enabling us to determine eligibility for our programs. Key Responsibilities: Conduct face-to-face meetings with IRIS participants to complete the screening process. Complete the Adult Long Term Care Functional Screen (LTC-FS) in accordance with Wisconsin Adult LTC-FS Instructions. Verify screen results through contact with IRIS Consultants, Medicaid Personal Care agencies, and, if necessary, validate diagnosis information with physicians and the Social Security Administration. Adhere to stringent documentation standards and program regulations, completing screens in a timely manner. Enhance screening skills by attending weekly team meetings, monthly All Screener Meetings, ongoing trainings, and assessments. Fulfill additional responsibilities as assigned by management. Qualifications: Required Education: Bachelor's Degree in a health or human services field (e.g., social work, psychology) or graduation from an Accredited School of Nursing. Required Experience and Skills: At least 1 year of experience working with target groups such as adults with physical or intellectual disabilities or older adults. Exceptional written and verbal communication skills with a keen attention to detail. Proficient in Microsoft Office Suite and other relevant software. Strong customer service and organizational skills. Good time management capabilities. Able to work effectively as part of a team or independently. Flexible in adapting to changing organizational needs. Willingness to travel as necessary (5%). Licensure and Certification: Must pass Wisconsin Adult Long-term Care Functional Screen certification modules (achieving 80% or higher on each) within the first week of employment and maintain certification. If graduated from an Accredited School of Nursing, must hold an active, unrestricted Wisconsin Registered Nursing (RN) license in good standing. Must have a valid driver's license, maintain appropriate auto insurance for travel, and be able to travel throughout Wisconsin. Preferred Qualifications: Certification in the Long-Term Care Functional Screen is a plus. Molina Healthcare offers a competitive benefits and compensation package.
