Travel RN Case Manager – Jackson, MS
Start Date: 10/20/2025 Duration: 13 Weeks Setting: Acute Care Shift: Days, 8-hour shifts (M–F 8:00 AM – 4:30 PM) Guaranteed Hours: 40 hours/weekWe’re seeking an experienced RN Case Manager for a 13-week contract in Jackson, Mississippi . The RN Case Manager will coordinate discharge planning for acute care patients across all inpatient units, ensuring safe transitions of care while collaborating with multidisciplinary teams.
Experience: Minimum 2 years of Acute Care Case Management experience, including at least 6 months in a Level 1 Trauma or large academic medical center.
Licensure: Active RN license (compact accepted). Willing to wait for Mississippi license if needed.
Certifications: BLS required; CCM preferred but not required.
Education: BSN preferred; ADN accepted.
Charting System: EpicCare Inpatient EMR (2014 Certified EHR Suite) — EPIC experience required.
Department: Care Management – Discharge Planning
Caseload: 15–20 patients/day
Scope: All inpatient units
Team Structure: 15 Case Managers (expanding to 21) and 30+ Social Workers (full-time and part-time combined)
Primary Focus: Medical discharges (some social discharges)
Tools: Currently using NaviHealth, transitioning to Epic Care Management within the year
Flu Vaccine: Required (medical exemptions only, with documentation).
Schedule: Monday–Friday, no weekends. Holidays rotated as needed.
